Patient Combine and Save Account

If you have a PKU prescription and would like to take advantage of FREE shipping for your Low Protein Food, please see the steps below to create an account.

Step 1 - Complete a Patient Combine and Save Account form. This can be completed online below or the PDF can be printed then e-mailed to info@cortexhealth.co.nz. - Print PDF

Step 2 - Wait for a notification that your account has been approved.

Step 3 - Log onto our website with your new Combine and Save account and purchase your Low Protein Food through our website.

Step 4 - Order your formula through your local pharmacy and let them know to link both orders will be sent to the pharmacy.

Please see your treating clinician / dietitian to discuss whether our products are suitable for you and to arrange a prescription.

Prescriptions can be dispensed through your local pharmacy.

⚠️ Pharmacy ordering update

From 1 April 2026, all pharmacies in New Zealand can order Cortex PKU products through CDC Pharmaceuticals, our New Zealand distributor.

If your pharmacy does not have a CDC account, your regular wholesaler will be able to order the stock directly through CDC.

There are no delivery costs associated with ordering our PKU formula.
